Immunization Record

 The State of Minnesota requires that all students provide an immunization record, unless they were born before 1957 or they graduated from a Minnesota high school since 1997.

Students who are not exempt must provide current vaccinations or exemptions, or their ability to register for classes will be blocked.

IMPORTANT: Your Diphtheria/Tetanus vaccination must be within the last 10 years to be current. Also, you must have at least one MMR dose at or after 12 months of age. If either of these are not current, your registration may be blocked.

A copy of the immunization record form is attached below.

This form should be returned to the ATCC Welcome Center.
